Hi,
Try the following, it might help you.
Go to Banners-->Delivery Options--> choose "Time-Date" option from "Add Delivery Limitation" and click on "Add" button. Do it twice, then follow the below :
In first Time-Date field, choose "Is later than or equal to" option and set date as "01 October 2014".
In second Time-Date field, choose "Is earlier than or equal to" option and set date as "31 October 2014".
Do the above and save changes.

Think I got it. It seems my banner size is 729 x 91 (don't know why it exported from the graphics program that size). I tweaked the banner size field and now it shows up.

Yes. The error is caused by the fact that the Cookie header is too long. In this case, OXLIA is getting too big: it is a cookie used for conversion tracking. Add that to the other cookies for frequency capping/blocking and you can easily get this overflow situation. Please try to reduce the number of campaigns with capping and/or conversion tracking.

I got it working. Here's what I did:
Working as administrator account, go to Plugins
Find "Delivery Limitations Plugin" - Click on details
Find "Client Delivery Limitation Plugin" - click on settings
Check the box beside "Extract the viewer's operating system and web browser information using phpSniff" to select it and save changes.
Now, go back to the banner you don't want to show to mobile devices.
Go to "delivery options"
Find "delivery limitation" - choose "client - useragent" and "add"
For "Client - Useragent" choose "regex does not match"
In the field below it, add the devices/browsers etc. that you want to exclude.
Example:
Android|iPhone|iPad|iPod|Mobile|kindle|Nexus|Opera Mini|IEMobile|BlackBerry|webOS
Click "Save Changes" and it should show the banner to desktops only.
There could be a better way, but this is working for me.

The solution was not to put the URL, but the server path into the config area in revive IP2Location plugin settings.

Your PHP setup might not allow Revive to redifine the include path. Maybe set_include_path is disabled via your php.ini?

There is a "help" link and a "support" link. The Support link can be configured entirely to your liking in the User Interface settings. The help link/button can't be customized.
