Seeing Ad On Each Refresh

First time user...I set up some test environment and I'm not sure how to set up the invocation code.

 

 

I put the JS snippet from websites -> invocation code on to a test html file on nginx (same nginx that's hosting Revive) and I didn't see any ads.

 

I put the invocation code for individual zones in the html file and seems that 1 shows up every 10 or so refreshes.  I put 3 different zones invocation codes on the same test.html file and still only 1 of them shows an add every 10 or so refreshes.  I'm sure I'm doing something wrong...any tips?
