Are you still having issue with this, or you solved it? Otherwise, report back. I'm using cloudflare also, and it works fine, with basic settings, so I can help you troubleshoot.

This has been commited to code in PR 1339 - https://github.com/revive-adserver/revive-adserver/pull/1339 Not sure if it'll be approved, but at least commited ?
