Administrator Login loop on upgrade

Hello Community.

I've been struggling to upgrade my 4.1.4 install to 4.2.1. Following the instructions here https://www.revive-adserver.com/support/upgrading/ has worked great in the past but not today. I get to the second step in running the actual upgrade and I cannot login. I just get dumped back at the same page and asked for my login again.

Having spent too much time already on this, I decided to upgrade to 4.2.0 from 4.1.4 - and following the same instructions etc. worked fine. Then, trying to upgrade from 4.2.0 to 4.2.1 I get the same issue - login loop.

I'm certain my .conf.php file is pointing to the correct database with the correct login. I'm confident the permissions have been properly set on the various folders etc. and I'm confident I have the correct admin password etc. to login to the Ad Server so I am at a loss. Hoping someone has a hint or clue which will help!
