Iframe Sometimes Doesn't Load

Hello my banners sometimes don't loaded.

I'm use a iframe to implement the banner

this is my code banner:

when dont load iframe the code this is:

Any ideia why iframe dont load?

I'm discovered when de banner don't load i receive the message

 

"NetworkError: 404 Not Found - http://example.com/www/images/a4f264ac30a5c38e0b71840c2606a28e.png"

 

I did reload and image loaded, but sometimes image don't load

So, to me, if it works sometimes and not other times, that suggests a transient network issue, or a load/capacity/threading issue with your server...
