It isn't. You should use the front controller delivery/fc.php for delivery files provided by plugins.

PHP 5.4.4 is not supported by Revive Adserver due to a bug fixed in later versions. Please have a look at our requirements page.

I apologise for that. There has been a glitch when packaging the release files and the last minute fix to the delivery cache plugin was not included. I have repackaged the release files and re-uploaded them.
Alternatively, you could get the fixed plugin from https://github.com/revive-adserver/revive-adserver/blob/master/etc/plugins/openXDeliveryCacheStore.zip?raw=true and put it in the etc/plugins folder right before retrying the installation from scratch.

The closed source ant library takes care of removing unwanted files and regenerating the delivery files. The input for file removal can be found here:

Actually, regenerating delivery files is taken care via a php script now. That has probably changed years ago without me noticing. So the only task performed by the library is making sure that the package files don't include unwanted files/directories

You are using a "layer" / interstitial invocation code. Pinning the ad to a corner is precisely what it's supposed to do.

Revive Adserver only supports VAST1.
At AdserverPlugins.com we have a plugin that can do VAST2 pre/mid/post-rolls.
